[Isis-users] Upgrade to Ubuntu 16.04 - problem with php script

Good evening,

This week our Ubuntu server was upgraded from 12.04 to 16.04.

After opening a database and opening data entry, I get the following
error message 

Notice: Array to string conversion
in /opt/ABCD/www/htdocs/central/dataentry/menu_main.php on line 12

php version is:  PHP 7.0.15-0ubuntu0.16.04.4 (cli) ( NTS ) Copyright (c)
1997-2017 The PHP Group Zend Engine v3.0.0, Copyright (c) 1998-2017 Zend
Technologies     with Zend OPcache v7.0.15-0ubuntu0.16.04.4, Copyright
(c) 1999-2017, by Zend Technologies 

And then the bar with the tool buttons (the arrows, searching etc.), is
not accessible any more.  see screen shot.
please advise what the problem could be.
